District Nursing Complexity and Dependency Tool, Tameside and Glossop Integrated Care NHS Foundation Trust

Our District Nursing Team works extremely hard to deliver high quality care in the community to the population of Tameside and Glossop. However, the District Nursing Team were reporting pressures on their service due to increased demand, which was compounded by:

  • Increasing pressures as patients with complex needs were being discharged earlier from hospital
  • the need to carry out more detailed clinical documentation / assessments for the ageing population
  • Inability to evidence of the increased demand to our local commissioners
 
Working in partnership with Corporate Information the District Nursing Team wanted to take an innovative approach to redesign their service. The ambition of the project was to create a level of detail and insight which highlighted changes in complexity, to facilitate safer caseload management and demonstrate additional resource requirements in terms of staffing numbers and skill mix. 
 
We met with clinicians to identify key factors that affect levels of complexity in patient care. Based on clinical guidance the key factors were given a score and categorised into 5 sections ranging from 'Minimal Complexity to Very High Complexity'. The results of the redesign allowed the Complexity and Dependency Tool to be integrated into the Trust EPR and was mandated to be populated at every new patient assessment to allow early identification of complexity level. This ensured that staff with the right skillset provided the care at the right time to ensure the best outcome for the patient. By using the tool, and complexity scores, we are able to monitor a patient's progress, and are aware if a patient's conditions are becoming more complex. This allows us to have 'person-centred' conversations with patients and other professionals.
