Complex Cases Team - Implementation of The Care Needs Test Pathway

The Care Needs Test Pathway is a MDT approach that ensures patients complex health needs are identified and the appropriate level of care is provided upon discharge from the Acute setting. The main principle of this pathway was to not delay care, ensure correct level of service provision and ‘to get it right first time’. This has enabled a smooth transition of post-acute needs to be continued into the community for long term management thus ensuring that patients with the most complex health needs are managed appropriately and safely.
